To print their immunization history, customers can log in to their Rite Aid pharmacy account. To request copies of prescription and immunization records, customers can use the online form on the third-party Medchart portal.
If a customer loses their CDC-issued COVID vaccine card, they can request a record at the pharmacy or online if they have an account set up. The CDC notes that it does not keep vaccination records and that Rite Aid will no longer be requiring vaccine cards.
